Joan Kwiatowski

Joan Kwiatkowski is currently the Chief Executive Officer of CareLink, Inc., a nonprofit health management services organization and The PACE Organization of Rhode Island. CareLink is comprised of post acute and community based health providers, creating an integrated network of services across the state. The mission of CareLink is to develop quality and cost saving opportunities for its members, while providing oversight to an integrated care coordination network for its members. Some of Joan's responsibilities include contract negotiations for managed care arrangements, oversight to a large group purchasing program, oversight to two profitable subsidiaries- a rehabilitation company and a mobile dental program and the development of the first PACE site in Rhode Island.
Joan serves as CEO of the PACE program, which is an integrated care model that serves frail elders who choose to remain living in the community and clinically qualify for institutional care. Joan participates in many state and national efforts focused on integration of care, chronic disease management and the development of new business models in health care.
Joan has served on the faculty at Boston University in the School of Social Work. Joan has published in the areas of case management, integration of health care and community and hospital collaborations. She has presented at many national conferences on related topics. In addition, Joan has held adjunct professor status at Tufts Medical School where she was clinical manager for the Behavioral Health Division and Geriatrics at the Family Practice Residency Program affiliated with Northeast Health Systems.
Joan graduated from Boston College with a degree in Psychology and completed her Masters in Social Work at Boston University. She completed a Certificate in Advanced Graduate Studies in Nonprofit Administration at the Stanford School of Business. Joan has served on a number of state and national nonprofit boards. She is married and mother to one recent college graduate and a current college student.
